Volunteer's Information
A Social Security Number is required to conduct the background check on potential volunteers. This information is collected and housed securely, and is only used for the purposes of a background check. A credit check is not done on any potential volunteer.
A Driver's License Number is required to conduct the background check on potential volunteers. This information is collected and housed securely, and is only used for the purposes of a background check. A credit check is not done on any potential volunteer.

Fair Credit Reporting Act Notice and Authorization

Notice
In order for National Heritage Academies to evaluate me for possible volunteer assignment, and if I am approved to volunteer, to make future decisions concerning my volunteer assignment, National Heritage Academies may from time to time obtain a consumer report about me.

Authorization
I hereby authorize National Heritage Academies from time to time to obtain a consumer report about me for volunteer assignment purposes. I understand that if my application is denied on the basis of information contained in a consumer report or if an adverse action is taken against me regarding my volunteer assignment based on information contained in a consumer report, National Heritage Academies will provide me with a copy of the report and a description of my rights under the Fair Credit Reporting Act.
